"I met with Dr. Jobin for bilateral shoulder pain. I fractured one shoulder. The bone became crooked. He referred to the displaced bone as "varus collapse." I have rotator cuff tears on that side that he describes as "full thickness" and "high grade" in contrast to the MRI report that all other doctors who viewed it agree with. He wrote in his after-visit notes after the 1st appt that he considered rotator cuff repair of tears as one of three treatments he considered but rejected it because it would leave 50% of pain due to the shifted bone. Due to the condition of my rotator cuff he decided reverse shoulder replacement surgery is needed. I learned only in his after-visit notes a single thing about his considered treatment options. Not one word in the appt. He used the letters "TSA" and "RSA." I had to look them up to have a clue what he referred to. A doctor is supposed to explain a patient's condition and treatment options in an appt. He mentioned other injuries in his notes that I'm clueless about. He asked me to return to discuss the other shoulder also injured and I did but he refused to discuss it. Wouldn't answer questions, just pushed RSA for the fractured side despite being the most invasive replacement surgery with many risks. He wanted to perform it immediately. Nothing that goes before surgery in a doctor-patient relationship occurred. He commented in the 2nd appt that he could tell I'm not ready. I couldn't be more ready for explained treatment. There has to be a rapport and some connection between doctor and patient and a patient needs to learn of risks and benefits to know what they're getting into. I saw another doctor for a 2nd opinion, because of which Jobin divested. A 2nd opinion is always helpful especially when the views are divergent which spurs a patient to think more about the best option. I returned for the 2nd appt he requested as I was hopeful that Jobin would tie everything together. I prepared questions. Jobin wouldn't even write visit notes for 2nd appt he was so disconnected after knowing I was considering another option. Wouldn't say a word about MRI on shoulder yet to be discussed, the reason he wanted me to return. The doctor offering a 2nd opinion disagreed with Jobin about RSA and said it would harm me. What Jobin did for notes for 2nd appt instead of documenting what occurred in a discrete note is merely copy notes from 1st appt and passively aggressively add meds I tried and had adverse reactions to, some negatively life-altering. He recast contraindicated/inefficacious meds as a refusal to take them. Jobin came under scrutiny for his cavalier treatment suggestions and refusal to have a discussion with a patient. To try to whitewash his urging for RSA, absent explanation and justification, he redid what he put out as notes for the 2nd appt, eliminating his recommendation for RSA, adding after stating the meds that are bad for me THAT I AM CONCERNED ABOUT ROTATOR CUFF BEING INADEQUATE AND WANT TO DISCUSS RSA as if I want the maximally invasive procedure, and he has nothing to do with the idea. He sought to distance himself from his position and encouragement to undergo RSA since the 2nd opinion considered it dangerous. He removed his prior mention of RSA too from his revised notes. It was a whitewash. RSA leaves no recourse if something goes wrong. You can't trust him. He could have explained my injuries and treatment options he mentioned in notes if he were willing. I discussed my fear of it based on what I read with the 2nd opinion doctor who agreed. Jobin is right that repairing a rotator cuff will not correct a malaligned bone and I believe his assessment of the cause of pain is correct. What was the basis for considering him was overshadowed by his lack of communication and lack of caring and demonstration that he can't get into any detail or establish a doctor patient relationship. Just doesn't care enough. I never had a chance to ask about a less invasive approach. I'm left with the chronic pain and severe limitations. Jobin is not a patient advocate but someone to fear."
