Please note: the SURVEY that I previously completed is a totally flawed instrument, and quite annoying to fill out because of its clunkiness. Three times it asks how likely it would be that I recommend the provider to a friend or family member. The earlier version (dropped about three years ago) was much better. It was much more focused and related to one's actual experience. It gave you the option of leaving your name, etc. I suggest Columbia Doctors go back to the earlier version."
