"Please visit this facility IN PERSON prior to sending a loved one here. This means also in the area where they are to be '"cared for". When choosing a rehab facility this was on a list the hospital provided. It was one of the highest rated and it was very close to our home. How it was so highly rated is beyond all comprehension to me. As others have previously stated this place is absolutely TERRIBLE!! Their pictures online are quite deceiving, not to mention the god awful stench this place has inside. It reflects the lack of cleanliness throughout the facility. My father in law was transferred to this place in November. He had broken his arm and already needed a walker hence the need for rehab. He has an ostomy bag and when he got there they had no supplies whatsoever to care for someone with this. We had to drive 40 min away to bring them from home for the entire stay. The people attending him as (I would not mar the title of nurse) had never dealt with someone with a bag and it clearly showed. At one point we were told that he became severely dehydrated. This was at around 1pm. I went over the next day at lunch and he STILL did not even have an IV yet. Turns out they have to use an outside person to do IV'S she finally got there and it had already been 24 hours. Unacceptable!! Every single day I had to bring him food since the food here is a downgrade from a prison meal. Multiple occasions I came in and there was dried feces on the bed and feces covered towels along with medical supplies strown about the floor. Pictures below for that. At the end of 2 weeks and zero improvement and health worsening. I called 911 and got him the hell out of there. Upon admission at the hospital it was found he had a severe UTI and worse the broken arm now looked like a question mark. He ended up having to have surgery the next day to attempt to correct it. Not one time in the rehab facility did we see a doctor. Talking to the ones they have hired as "nurses" is like talking to a wall. I certainly question their medical education. They are also wayyy understaffed call lights go unanswered for hours. How this place has not been shut down is beyond me. Much more to complain about, but I think I have made my point clear."

The "Ritz Carlton" level of care here is driven by the Fair Oaks leadership team, which treats residents like family. Special thanks to Rhonda Lewis (Social Services) and Nicole Tangwa (DON) for their compassionate leadership, and Rebecca White for making families feel so supported. The clinical care is top-tier; Nurse Practitioner Meghen Anezin is incredibly smart and dedicated, while Talia Hanley and the Fair Oaks Administrator Felix Nwogdo go above and beyond daily to ensure resident comfort. It's rare to find a skilled nursing team that always goes above and beyond for every single resident--highly recommended! For anyone looking for a SNF for their family member you would be fortunate to have your loved one at the Fair Oaks Skilled Nursing facility."

"This review is specifically for Rhonda . There are alot of good things you can say about any social worker but Rhonda ticks every checks boxes you can think of. She has unwavering dedication not just at her work but as a person outside work. One thing you need to be as a social worker is the ability to foster trust, ability to project empathy, ability to create a safe, non-judgmental space. Rhonda is all that and much more. I have worked with her for a very long time and I can tell you the experience is something you will cherish. Highly recommend."

"I have been a longer than expected patient resident at "FOHR" in their PARC unit (Post Acute Recovery), so I have the 'benefit' of some time and experience with the facility and staff. I can't go through all the individuals as there are too many to name, so I will say with sincerity the entire staff has been outstanding, with special recognition to the nurses, nurse practitioners, aides, supervisors, unit managers, case workers, and physical and occupational therapists (you know who you are;-). My long and complicated recovery has been both facilitated and made more tolerable by this Skilled Nursing Facility. Highly recommended if you are seeking quality care in northern Virginia. "I won't go into great great detail about our exact issue but I would NOT recommend this facility to anyone who is not able to do for themselves to some degree. My brother passed after his care received here. He should not have passed as he had no complications from surgery before arrival, had no other unrelated health issues outside care needed for his injury and from surgery (which was why he went to the facility) prior to the facility. He was expected to make a full recovery in a few months and finish recovery at home. In general 3-6mo facility and another 3-9mo at home to be good as new. We were told this facility was equipped to handle his needs (this was a boldface lie). I have a lot to say but I believe I may seek legal counsel bc no family should go through this and it should not be allowed for patients to be neglected so they can cash checks from insurance and patients alike, so will refrain from listing the details here because this absolutely should NOT have happened and could have been avoided if he had received proper care. BUT I WANT TO INFORM OTHERS so you do not go through what our family did. If your loved one cannot advocate for themselves verbally and have some movement out of the bed or wheelchair on their own DO NOT send them here as you are only signing them up to be neglected. We chose this facility because we were told they were equipped for his needs and care. Not exactly true and not the care one would expect - additionally the night staff is mostly a joke. They run a very very very Skelton style staff that's honestly unacceptable just bc you believe patients should be asleep doesn't mean care and attention is not needed. Call buttons or go ignored for extended period of time. You must stay there at all times with your loved one to get care they need which is nearly impossible to do and should not be the case or required since this is the actual point of a care facility. I understand they say they aren't 1 on 1 but if your patient cannot speak up for themselves verbally or push the call button you can't rely on every 2 hours alone and wait for the family to show to call for help and request the physician and 911 due to their lack of attention and care overnight and even during the day, still having to push and request for care and attention and request management. And yes some of the staff was wonderful but not everyone has the same care and attention to care and work ethic. Wish they did but they don't and I don't accept that that's not the expectation when you are in this line of work. Unfortunately it appears people are hired bc they need a job or to fill a role not bc it's love of a career choice. And I'm not saying this is an easy line of work, it's hard and likely hard emotionally, mentally and physically BUT you are paid for the job, you have chosen to accept this line of work, and you are literally entrusted with someone's well being. I'm sure they would feel differently if it was their loved one receiving care by themselves or their coworkers and management. AND NO, I DON'T NEED YOU TO RESPOND TO MY REVIEW ASKING TO TALK AS I'VE SEEN IN THE OTHER RESPONSE COMMENTS. What good does talking do me now that he's passed? You should have been proactive from day one 24/7."
