"My father stayed here for 1 month for rehab and I only have good words to say about the staff. From the Nursing, Rehab, Speech, and Social services dept, everybody was very helpful and accommodating. My dad got a private room which was very clean and well maintained. His progress was very quick due to the best care he got from the staff. Shoutout to Jerome from Social services for helping him setup all the post rehab care he needs. Thank you again to everyone "

"Choosing a nursing home is never easy, but this facility truly exceeded our expectations. The staff went above and beyond to make sure my loved one was comfortable, safe, and treated with compassion. From the nurses to the support staff, everyone was professional and kind. The facility was always clean, and concerns were addressed quickly. We are very grateful for the care provided and would not hesitate to recommend this nursing home to others."

"My mom was in this facility before Covid and during it. Prior to Covid, short staffing, nobody responded to her bathroom calls. Which she need assistance. Nurses were aggressive, not sympathetic to her dementia. She fell a few times. Wanted to bring her home when it was time, then Covid and couldn't do anything. This was 2019 / 2020 She passed away on May 14th 2020. Maybe there is better staff now. But at that time, TERRIBLE STAFF AND WRONG PROCEDURES. My mom had to suffer nursing abuse and Covid. She was a wonderful woman who didn't deserve any of this. When I drive by Meadowbrook, I get sick to my stomach. TERRIBLE place."

"My grandma was brought here for hospice and Meadowbrook was nothing but accommodating. They did everything they could to make sure she was comfortable in her final days. They even gave her a haircut and allowed for her cat and dog to go to say their goodbyes as well. I would definitely recommend Meadowbrook."

"My husband/dad/brother has been a resident at Meadowbrook in Bolingbrook for 15 months. He has many needs requiring lots of specific care and attention. Over these months we've worked daily with staff and leadership to meet his needs consistently. Our collaboration has made us confident in several things: 1) the staff are good people and some not only do their best but go out of their way for the residents; 2) Meadowbrook faces challenges that all rehab/long term care facilities do (e.g. staffing, resources). But despite these, the new management is having a positive impact on staff morale and patient care. This leadership is seeking to create a positive culture where high performance and safety are priorities. Though some inconsistencies occur when new staff come on, leadership responds to our concerns. Meadowbrook team, we are grateful for you!"

"Well my mother was brought here for rehab after a hospital stay. They were told that she is a fall risk upon arrival. The first night she was here after we left, she was taken to the common area to watch tv. Well, she got up out of her wheelchair and fell to the ground and hit her head! She was taken to the hospital for a CT scan, luckily it was negative. There was allegedly a CNA watching her but apparently not close enough to stop her from falling. I am not happy that she is here. Rooms are old and dingy and NO TV in the rooms. I had to buy a TV for her for her stay here. Hopefully she will be out soon."

"Meadowbrook Manor took care of brother who was in Hospice. I can not Thank Them enough for everything they did to make his final days peaceful. My heartfelt Thanks to everyone at the facility, they would call me with with how he was daily, as I live in Indiana and couldn't always make it in to see him. Thank You, Meadowbrook made a very difficult time somewhat easier to deal with."

"I would like to personally thank Moneka, Ana, Felicity, Irma, Dan, Mahira, Lauren, Joy, Barb, Jacky, and all of the Pavilion staff for the exceptional care they provided. My grandma, even at 86 years old, was one of the most independent women you would ever meet. After suffering a stroke and being placed into hospice, relying on others was not something she would have wanted. However, if she had the ability to speak and express herself, I know she would be deeply thankful for every single person who cared for her. The staff treated her like a human being, not just a patient or a bed to tend to. They played music for her, cared for her with dignity, bathed her, changed her, and even gave her a haircut after the hospital left her hair severely matted. These acts of kindness may seem small to some, but they meant everything to our family. They made an incredibly difficult situation a little less painful, and for that, I am forever grateful. I cannot praise this facility and its staff enough. From the bottom of our hearts, thank you."
