"In response to my earlier review, staff came to ask me what could be done. I showed them the bathroom floor. This was FOUR days ago. Nothings been done. I am discharging this morning and was given major attitude when brought up to the nursing staff. Pardon me for asking you to do your job. I will keep in mind this facility if I ever go into nursing and plan on sitting on my behind all day. This does not apply to everyone however, it does to about 90% of the staff at this facility. The nurse who handled my discharge, Davis, was EXTREMELY rude to not only myself but other patients who were awaiting medication. The tone in her voice was condescending and dismissive. Not a way to speak to residents."

"Staff is very rude and rushed I understand you have a lot of paitents to look after but to rush someone when they are to weak to even move gives to impression of no compassion. Also staff needs extensive training on how to talk and listen to people concerns with out making them feel like they are mentally challenged. The place needs adiquite cleaning I never been somewhere that smells so bad of urine. Equipment & furniture very outdated to the point of falling over. I pray no one will ever have to go to this place. We definetly plan to move our love one right away."
"For anyone that has a love one here, please I beg you to check on them. I wouldn't trust the nurses if my life depended on it!!! They have some nurses who has no respect, love, honesty, or integrity. Your love one may die at the hands of the nurses purposely!!! If you LOSE someone get an AUTOPSY!!! A video is circulating with some nurses and their family from there that attacked a 74 year old Grandmother in a home invasion. I pray to God they're prosecuted to the fullest extent!!!"
"I would not recommend this facility to no one. After having my love one there 2days this has been my observation...The staff is unprofessional, lazy and don't have passion to work in this capacity. Staff treat the facility as pay check only and not client focused atmosphere. They show no interest in the clients well being. After visiting hours the staff gather and chat all night at the front desk while the clients that are wheel chair bound roam in and out of other clients room unsupervised. During the day the staff does no interacting with the clients (4th floor) they sit them in a common area room and leave them to ALL watch one 42inch tv. If you have a client that need assistance to be feed that client will not get the attention that's needed to eat. I'm highly disappointed in the services that's provided to my love one and has taken steps to get them transferred or brought back home. I recommend this facility to be shut down or throughly investigate by the state of Illinois. DO NOT make the mistake I did and bring your love one to Symphony of Chicago West. It's a sad state of affairs that we can't have quality care in our community."
"After 10 years as a resident, Symphony decided to put an unauthorized DNR on my sister's file. All week, after finding out from insurance about it, My sister and I has told Symphony repeatedlyshe wants it removed. She wants to be resuscitated. Symphony refused to remove it, taking away her rights. God forbid something happens this weekend. I am contacting ILDPH abuse hotline tomorrow. Symphony doesn't get to decide life or death for my sister."
"When was there for two years I developed stage IV pressure ulcers in 2005 which I still have! It's a beautiful place but the staff are there just for a job. They are ghetto to the utmost! The rehabiltation staff are excellent when I was there but the doctors are non knowledgea I was able to walk after head trauma. They would get you up at 6:30 in the morning and wouldn't put you back to bed till 6:30 pm. They had insufficient staff who were over worked an under paid! Since Symphony bought them I hope things has changed for the better!"
