Avantara Evergreen Park

"I selected Avantara because of it's location and proximity to my house. We needed a Rehab facility for my brother who had fractured his tailbone. I was very impressed with the care that he received while at this facility. Stephanie, the Social Worker, was extremely helpful in making sure that my brother's stay there was pleasant and beneficial. We also met with the business manager and the nutritionist, and I sat in on one of the physical therapy sessions, which was very effective in helping him to progress from his injury. My brother needed to be transferred to an assistant living facility from Avantara and Stephanie was a Godsend in helping to facilitate this. He also needed a hospital bed and she was able to secure that as well. I would highly recommend Avantara."

"My stay here was really while to doctor and the nurses was on point. If someone is looking for a place I would recommend this place. The nurses and the class is very helpful. Physical was a success i feel very confident going home. In the beginning I was nervous this is my second time coming and both stay was great. They staff encourage you to be better . I appreciate everything they did for me I thank God for this place."

"If a zero-star rating were possible, Avantara Evergreen Park would deserve it. If you truly love someone, do not leave them in this facility. I understand no place is perfect and many healthcare facilities face challenges, but this one crosses the line in every way imaginable. Communication is non-existent. Instead of addressing concerns, staff avoid family members--literally turning and walking away. There is no transparency, no compassion, and no urgency to resolve serious issues. Severely understaffed and mismanaged. There is a 1:22 nurse/CNA-to-patient ratio, which is absolutely unacceptable. The staff are clearly overworked, yet often seen doing anything but patient care. Many work excessive double shifts just to keep the facility running, but it's still not enough. Unsafe conditions -- contaminated water! We were never informed when the facility's water was deemed contaminated. I only found out because my husband wasn't receiving his scheduled baths. When he asked for one, he was told it couldn't happen due to contaminated water -- yet no one contacted me or any family members. This is negligence. Poor leadership from the top down. The head administrator, Neely Howard, never responded to multiple voicemails I left. In my husband's entire 4-week stay, she never once spoke with me, despite repeated requests. This is unacceptable and shows a complete lack of professionalism. Therapy department is disorganized and unmotivated. The Director of Therapy is routinely late, disengaged, and full of excuses. Important medical documentation to lift my husband's weight-bearing restriction was ignored for days, delaying his recovery -- and only addressed because I personally followed up and had the hospital resend it. The physical environment is also poor. The facility smells, the interiors are outdated and unattended, and many staff display attitudes that show they clearly do not want to be there. This is especially disheartening when some staff are genuinely trying under impossible conditions. Final Thoughts: Avantara Evergreen Park needs a complete overhaul--from staffing and management to safety protocols and communication. These are people's loved ones, and often the only caregiving option they have. Until drastic improvements are made, this facility should not be trusted with anyone's care. Do better--or close your doors."
